How does Club Evolus work?
- You will be charged $0 at enrollment, and then $49 recurring every 30 days.
- You will be eligible for your first treatment after 3 payments of $49. There is an option to prepay at enrollment to get your first treatment as early as that same day.
- Your Club Evolus membership covers 20 units of Jeuveau per treatment, with a minimum of 90 days between each treatment.
- Units do not accrue if you wait more than 90 days between treatments.
- To maximize your savings, get treated every 90 days!
- You will pay your provider directly for any additional treatments you receive.
- Cannot be combined with any other offer.
- Club Evolus members will be automatically opted out of Evolus Rewards upon enrollment.

How do I enroll in Club Evolus?
Prior to enrollment, please make sure there is a participating practice in your area.
How do I enroll?
Go to this site: https://club.evolus.com/ and select 'Join the Club' to create a profile. You can also enroll at a participating practice.
How do I make my first appointment?
Reach out to directly to the practice you are enrolled at to make your first appointment.
When can I get my first treatment?
Depending on what membership option you choose, you may be able to get treated as soon as the same day you enroll or 90-days from enrollment.

You can sign in at club.evolus.com to view your next eligible treatment date. You'll be eligible for your first treatment after 3 monthly payments. We also have a membership option that allows you prepay to receive your first treatment as early as the same day you enroll.

How do I make Club Evolus payments?
What payment methods are accepted?
Any accepted credit card provided on club.evolus.com
How do I update my payment information?
Payment methods can only be updated by signing in at club.evolus.com. Once you go into the Membership tab menu, click on the 3 dots next to Billing Information. You can update and save a new form of payment for your next billing cycle.
What day of the month will I be billed?
Every 30 days from the day of enrollment.
How will the payment show on my credit card statement?

How do I make cancel or make changes to my subscription?
Can my membership be frozen or paused?
No, Club Evolus memberships cannot be frozen or paused.
How do I cancel my membership?
You can cancel at any time by signing in at club.evolus.com. You will not be charged on the next billing. Any unused treatments will be lost.
Can my memberships be transferred to another individual?
No, memberships are non- transferable.
Will I be able to get a refund if I cancel?
No, all payments are non-refundable.
If I cancel and want to re-enroll, what is the process?
You will need to log in to club.evolus.com, go to the membership tab, and reactivate membership.
Is there a cancellation fee?
If a membership is cancelled within three months of enrollment, a $100 cancellation fee will be applied.
How quickly do cancellations go into effect?
Immediately. You will not be charged on the next billing cycle.
