  • I am a medical provider. How do I place an order?

    Once logged into the Evolus Portal/Practice App, there are multiple places where you can place an order. We recommend starting with the My Offers tab. This will show you any offer your account is eligible for. Select Apply Offer and it will automatically apply the promo code. 

    You can also input a promo code on the last screen when placing an order. Don’t forget to select APPLY 😉

    Update my Password in the Evolus Practice App/Portal

    1. Select the hamburger icon in the top right-hand corner
    2. Select User Profile Settings
    3. Under Change Password, create a new unique password

    While logged in to the portal, please select the following:

    2. User Profile Settings
    3. Login Settings

    Please send us a few details HERE.  We will make sure that this event is reported.

    • The name of the practice and city where you received treatment
    • Your initials
    • Your date of birth
    • Your treatment date.
    • Treatment location and units treated.
    • Description of your symptoms (if any)

    We will contact the practice where you received treatment, and they will reach out to you soon.
